Paid Time Off Policy
Only for full-time employees
Lyquix believes that employees should have opportunities to enjoy time away from work to help balance their lives. Lyquix recognizes that employees have diverse needs for time off from work. Lyquix has established this paid time off (PTO) policy to meet those needs. The benefits of PTO are that it promotes a flexible approach to time off. Employees are accountable and responsible for managing their own PTO hours to allow for adequate reserves if there is a need to cover vacation, illness or disability, appointments, emergencies or other needs that require time off from work.
Eligibility
Only employees working an average of 30 hours per week or more are eligible. Employees that work an average of fewer than 30 hours per week and temporary employees are not eligible to accrue PTO.
PTO is accrued once the employee has been employed continuously for 3 months.
Accrual of PTO
The amount of PTO days is based on the employee's number of completed years of employment with Lyquix.
For new hires, once the employee has been employeed continuously for 3 months, the employee will accrue one day of PTO for each whole month remaining until the end of the calendar year.
On the subsequent calendar years, on Janaury 1st, the employee will accrue 15 days of PTO, plus one additional day for each additional completed year of employment, up to a maximum of 25 days of PTO.
Up to 5 days of unused PTO can be carried over the following calendar year.
Use and Scheduling of PTO
Whenever possible, PTO must be scheduled in advance for time off for vacations, personal leave appointments, or other reasons. It is subject to supervisory approval, staffing needs, and established procedures.
Unscheduled absences will be monitored. An employee will be counseled when the frequency of unscheduled absences adversely affects their performance or that of the rest of the team.
PTO is paid at the employee's straight-time rate. PTO is not part of any overtime calculation. Employees are required to use available PTO when taking time off from work with the exception of a company-required absence due to low workload or absences occasioned by the company.
PTO must be taken in increments of half workday. Unless approved by the employee's supervisor, employees are not allowed to be absent from work more than 16 calendar days in a row, even when PTO is combined with holidays and weekends.
Payment upon Termination
Employees will be paid upon resignation, separation, or retirement for all unused PTO days accumulated but not used in accordance with state law requirements. Employees whose positions are eliminated through a reduction in force or reorganization or are whose hours drop below 20 hours per week are paid PTO on the effective date of the termination.
